Payment Information

Please note that Empower Balance Therapy & Wellness is an “Out of Network” (OON) cash-based practice. This allows us to work for YOU without your insurance company dictating your care. For physical therapy, we can provide you with a detailed superbill at your request that you may self-submit to your insurance for potential reimbursement directly to you.

Our three specialty exercise programs, (Anti-Freeze Bootcamp, Jumpstart, and Neuro Performance) are cash pay programs and not covered by insurance.

If you are a Medicare beneficiary, you can receive the services that Medicare does not cover, including Anti-Freeze Bootcamp, Jumpstart and Neuro Performance. We cannot provide physical therapy to Medicare beneficiaries.

Payment options include cash, check, credit, Flexible Spending Accounts (FSA), Health Savings Accounts (HSA), and gift cards.
